Every person in Jackson County and the surrounding areas, in need of medical assistance due to his/her individual illness or injury, shall receive unconditional assistance to help promote for him/her a healthy, happy, and long life.

About Home Health:

Home Health is a service that must be ordered by your Physician.  You do not have to be hospitalized prior to a Home Health Admission.  You do need to be considered, "homebound" during the time of your illness or injury.  You may need to see your Physician/Physicians during you home health episode for regular visits.

Your home health nurse and Physician will communicate regularly to meet all of your health care needs.

We will provide as much teaching regarding your disease process, surgical recovery, or other illness as you require.

We are able to provide either long-term visits or short-term care visits as you require.

We offer two nursing assessments per calendar year to see if you qualify for Home Health visits. 

Feel free to call us if you would like a nursing assessment.  We will notify your Physician of your request.
