Just as it says, home health care provides services in the patient's home or place of residence. We believe that the best environment for healing is in the home.
Anyone who has recently experienced a change in medication or health condition, had a recent or past hospitalization, and has a difficulty leaving the home may qualify for home health care.
Common reasons for home care include:
Our agency will strive to offer you the tools that will help you or your loved one regain and retain optimal health.
Home health care is covered by Medicare, Medicaid and most other private insurances on a case by case basis.
Most health plans cover the cost of home health care. However, each plan may have certain conditions or criteria that must be met in order to qualify. Also, many insurer's require the patient to use a home health agency that is part of their provider network.
If you are interested in learning more or finding out if you are covered, please contact your insurance company, or our agency, and we will be happy to contact your insurance company for you.
Yes. By law, all doctors and hospitals are required to allow you to choose which home care agency you use.
No. There is no age range for client's of home health care agencies. Senior citizens are generally the majority of our patients, but many people who need assistance get home health care at any age for themselves, their parents, or their grandparents. It will all depend of your needs.
The services you receive are prescribed by your physician. In most cases, your doctor will recommend home health care. From there, a nurse is sent from the agency, and along with the doctor, will determine what services would benefit the patient the most.
The frequency of visits will depend on the patient's needs and the physician orders. A plan of care is written and discussed with the patient on admission. Usually visits range from 1-3, sometimes more, but this range will vary based on the individual needs of the patient.
Services usually start within 24 hours of Ann's Home Health Agency's receipt of the referral unless the physician has ordered a specific date for services to begin.
Home Health services continue as long as medically necessary, based on the assessment of the registered nurse, your physician and your progress.
You may be considered "homebound" is you have difficulty leaving home by yourself, require assistance to leave home, and do no leave home frequently, due to your medical condition. The nurse will determine your homebound status as part of the initial assessment visit.
In many cases, home health care permits a person to be discharged earlier from a hospital, reduces hospital admissions, and assists with a more rapid recovery. Most importantly, people are happier at home and receive more care and attention from family, friends, and home health care professionals.
